As your Lavender Plan project manager, I can work with you to create a comprehensive plan that includes sharing your priorities with your healthcare proxy and power of attorney for property, outlining your wishes for end-of-life care, and annotations. By doing so, you can make living in the present more enjoyable and less stressful for those around you while providing important tools for your family, friends, and executors.

Your Lavender Plan will consist of:

  • Letters for your family and friends to share your wishes for living, managing hiccups, and planning for how you plan to age gracefully.

  • A roadmap documenting the pieces you have in place for living your life.

  • A letter to your healthcare proxy giving details of your actual living wishes to your healthcare proxy.

  • A letter to your Power of Attorney for Agent (if you have one) providing your Agent priority instructions.

  • Distribution of desired communications.

  • Recommended worksheets in case of emergency for those who might need it.
